4. Materials and cost

Our volunteers give their labour for free. Parts are a separate question and we would rather know about it now than halfway through.

Materials will need paying for Lumber, a part, a fitting - something has to be bought.

Roughly what will it cost (optional) A guess is fine. Write it as a number, like 250.

What has to be bought (optional) What is needed, and how much of it you can cover yourself.

5. A photo

Consent, Waiver, and Release of Liability for Participation in Services Rendered

CONSENT, WAIVER, AND RELEASE OF LIABILITY FOR PARTICIPATION IN SERVICES RENDERED BY PROJECT ONE MORE MISSION

I, ____________, hereby acknowledge that I will be voluntarily receiving services from PROJECT ONE MORE MISSION of KINGSPORT, TENNESSEE.

In consideration of these services, I agree to save and hold harmless PROJECT ONE MORE MISSION, along with their respective officers, from any claim by me, my family, estate, heirs, or assigns, arising out of the services provided by PROJECT ONE MORE MISSION. I further waive and declare that I will hold PROJECT ONE MORE MISSION and their respective agents and employees, harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to me or my property, even injury resulting in death, which I now have or which may arise in the future in connection with the ministry activity referenced above or participation in any other associated activities. I also specifically agree that PROJECT ONE MORE MISSION shall not be responsible for any such injuries, loss, or damage even in the event of negligence or fault by PROJECT ONE MORE MISSION.

I understand and hereby agree to assume all of the risks which may be encountered while receiving services or sleeping overnight on the premises indicated above, including activities preliminary and subsequent thereto.

I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of TENNESSEE, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital.

No one has influenced or coerced my actions in any way. I understand that this is a legally binding agreement that I have read, understand, and voluntarily consent to. I further state that I have carefully read the foregoing release and fully understand the contents thereof, and I sign this release of my own free will.

AUTHORIZATION FOR USE OF COMMENTS AND PICTURES

PROJECT ONE MORE MISSION AND ALL AFFILIATED MEMBERS are hereby authorized to take or permit pictures to be taken of me, ____________, for use by PROJECT ONE MORE MISSION in publications, newspapers, newsletters, its website, and/or on television for purposes of public relations for PROJECT ONE MORE MISSION. PROJECT ONE MORE MISSION shall also have the right to use any portion of any statement made by me in any publication, newspaper, newsletter, or website.

This agreement contains all the understandings, oral and written, of the parties and supersedes all previous agreements. If any portion of this Agreement is found to be invalid or unenforceable, it shall not affect the balance of this Agreement. This agreement will be governed by the law of the State of Tennessee. I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Tennessee and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. The terms of this release are contractual and not a mere recital.
